Position Summary:
The Patient Care Coordinator is responsible for assessing, monitoring, and evaluating the patient care process. This position is a vital member of the patient’s care team.
Responsibilities:
· Coordinate abdominal KUBs and Gastrostomy tube replacements for urgent needs and routine replacements.
· Coordinate lab orders.
· Appropriate follow-up with pharmacies on medication orders.
· Coordinate with the Scheduling team to manage the provider's schedule.
· Maintaining and updating Wound Ally with the highest level of accuracy as needed.
· Monitor patient hospitalization lists.
· Maintain and updating schedules, calendars, and agendas.
· Assist with appointment confirmations.
· Efficiently retrieve consent and arbitration forms while ensuring proper storage of all documents.
· Sending out reminders of scheduled appointments.
· Answering scheduling queries via email and phone.
· Performing other administrative tasks when required.
· Monitor patient hospitalization lists.
· Ensure Durable Medical Equipment orders are processed.
Requirements:
· Must be a Registered Nurse
· A minimum of one year experience in a similar role.
· Advanced knowledge of confirming and rescheduling appointments and recordkeeping practices.
· Strong critical thinking skills and the ability to solve scheduling challenges.
· Extensive experience with office software, including MS Word, Excel, and Dropbox would be an asset.
· Excellent organizational, interpersonal, and communication skills.
Job Type: Full-time
Salary: Php27,500.00 per month
